Incident/Accident/Near Miss Reporting
Teesdale SWC uses the Accident/Incident/Near Miss Reporting Form to record incidents that happen at the Club and to learn lessons to reduce the likelihood of such accident occurring again in the future.
In preference, these forms should be completed while still at the Club on the day of the incident and be posted in the secure letterbox. However, there may be occasions when it is impossible/impractical to do so. In which case, the downloadable form below should be completed and emailed to: commodore@tdswc.org.uk
